Fill in the blank & e-signature. Cosigner Application is for the person bonding the defendant out of jail. Every blank must be filled out as it pertains to yourself, the only blank about the defendant is their name, phone number and bond amount. Please ensure you enter the bond amount correctly or it will have to be redone. If a blank doesn’t pertain to you please put n/a or 1’s for number blanks. The person filling the form must be done by the actual cosigner and the DL must be uploaded, we do capture IP addresses. The credit card area is manual -not automatic. EVERY application must have a card submitted – you can write $1 for amount of credit card authorization.
